Video Camera, Tripod, and Mic Kit (advanced reservation required)

This kit is reservable by UD students, staff, and faculty for UD-related purposes. Please note the following policies:

The kit is reservable on a first-come, first-serve basis
The maximum length of the loan can be up to seven days
Requests to borrow this kit need to be made 24 hours prior to pickup
Reservations can be made up to 12 months in advance
If reserved item is not picked up with 24 hours of the loan start date, the reservation will be canceled and made available for others to reserve
This kit may not be renewed
The person filling out this form must be the person who picks up the kit, and borrows it with their UD ID (OneCard) from the SMDC service desk
Anyone can return the kit, but you will be responsible for any late, lost, or damaged equipment
Reservable kits that are returned late will result in loss of kit reservation privileges
